Submitting a form

The individual to respond to a formClosed A form is a questionnaire to gather information. is notified by email to start the submission process.

  1. An email with a link is sent to the person to complete the form.
  2. Read the description and agreement supplied. If you agree, enter the User ID in the Email Address text box. This is the email User ID you received in the invitation email. Select Submit. Note that you may be able to change the language from English.
  3. On the next screen, enter the Password supplied in the invitation email.
    • If you lost the password, select Forgot your password? and one will be emailed to you.
    • If you want to change your password as a security measure, select Change your password and enter a new password.
  4. On the Your Questionnaires page, select the Continue button to complete a questionnaire.
    Note that you can search for a questionnaire using the Search text box.
  5. Complete the form then select the Submit button.
    • If there are required fields that you did not complete, the fields are highlighted in red for you to complete. Once complete, select Submit again.
    • Once a form has been successfully submitted, the button to Continue on the Your Questionnaires page is not available.
